The team provides support and treatment to service users in their own homes who are over the age of 65 and are experiencing the onset or relapse of severe mental distress.

The service operates across 7 days a week between the hours of 8.00 am and 8.00pm you will be expected to work 11.5-hour shifts for 3 weeks of the month and 4 shifts for one week of the month {full time}.
At the time of advertising, this role does meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK.
Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000, as required by the UK Visas and Immigration.
Main duties of the job
The successful practitioner will work within the remit of the band 6 Senior Clinical Practitioner job description.
You will be undertaking crisis assessments and formulating plans to support the service users to safely remain at home. You will require experience or understanding of positive risk taking and be able to complete documentation to support this. Where it is deemed unsafe to provide continued support at home you will be an integral part of acting as a gatekeeper for admissions to the ward.
A key element of your role will be your ability work both autonomously when making decisions in the community but also sharing knowledge of the whole team caseload with the multi-disciplinary team.
You will need to remain calm and resilient in a busy team demonstrating compassion in often highly stressful situations. Along side this you will be well supported by the leadership team and colleagues.
You will require an interest in Older Peoples needs and understand the need for a holistic approach to their care.
You will have strong and effective communication and engagement skills with service users, family, friends and carers.
Being a car driver is essential as the team covers a large geographical area.
