Rehabilitation Assistant Practitioner Full Time NEW

We are looking for an individual passionate about providing community rehabilitation, who has:

• Relevant experience in a health and social care setting and base level of Occupational therapy/Physiotherapy knowledge.
• Knowledge of patient/client conditions relevant to physical rehabilitation in the community e.g. frailty, care post hip replacement or managing deconditioning related to admission to hospital.
• Ability to work under own direction.
• Able to travel to meet the need of the role.

The successful candidate will work alongside the therapy staff to promote independence and improve function for people living in Waltham Forest.

Main duties of the job

As a Rehabilitation Assistant Practitioner, you will be responsible for an allocated caseload of patients and will assess, plan and deliver care.

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.

Detailed Job Description And Main Responsibilities

• To be responsible for organising and prioritising own workload on a day-to-day basis.
• To have organisational knowledge relating to Trust protocols and procedures and adhere to them e.g. lone working, moving and handling.
• To be responsible for providing accurate records of information required by the Trust for audit purposes.
• To ensure effective risk management at team level by accident/incident reporting, assessing, and controlling risk and ensuring residual risks are made known to the Clinical Lead therapist(s).
• To contribute to the education of pre-registration students with support from senior staff.
• To employ professional curiosity in your day-to-day role and act upon any findings appropriately.
• To undertake training as required by the role.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME)

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
