The Post Holder Will Be Expected To To proactively manage and liaise with clinical and administrative teams regarding all suspected and diagnosed cancer patients to ensure each stage of their pathway is fully planned and booked to ensure Cancer Wait Times targets are achieved. To work as a key part of the Multidisciplinary Team (MDT) to ensure the cancer patient and carer experience is central to every stage of the patient journey. Ensure the MDT meetings are run efficiently and effectively, are well structured, all cancer patients are discussed and the whole patient journey is recorded on the Patient Pathway Management (PPM) database. Participate across a range of functions including supporting Audit, the Cancer Reform agenda, National Service Frameworks and Clinical Governance. Work with external organisations to ensure effective communication across service providers for the smooth transfer of cancer patients.The Post Holder Will Be Responsible For Tracking patients and actively managing the clinical pathway to ensure appointments, investigations, MDT discussion and treatments are conducted in a timely manner consistent with their timed clinical pathway. Actively monitor cancer waiting times for individual cancer patients by proactively identifying risks. To take corrective action to ensure the national & local targets do not breach. Liaise with clinical teams to expedite, where necessary, any interventions which appear to be causing a delay or are outside expected timescales. Escalate to line manager / General Managers where there is a risk of breaches in line with performance for cancer waiting times. Ensure accurate and concise information is recorded on the PPM database. Liaise with respective Coordinators for other providers where patients have been referred for further investigations / treatments to ensure all necessary information has been transferred. The post holder will be responsible for the organisation of MDT meetings. Ensure room bookings are made and confirmed, and all equipment is working and set up in readiness for the MDT.
