We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.Please note that this post will not be eligible for sponsorship under the or routes.The Primary Care Team specifically are responsible for the direct delivery of the transformation of Primary Care Services, through key programmes of work including the Primary Care Implementation Plan, Unscheduled Care and other relevant initiatives.Key functions of the service include service development, contributing to service planning, delivering and supporting service improvement activities directed by the Primary Care Transformation Manager which falls under the remit of the wider Health & Community Care Directorate.Providing support, guidance and direction to a full range of services including Primary Care Contractors (General Practice, Dental, Pharmacy and Optometry), Advanced Nurse Practitioners, Adult Community Nursing, and a broad range of multidisciplinary teams and services. The team sits within the overarching Health and Community Care Directorate and are responsible for the delivery of a range of local and area wide services within Primary Care.The RoleThe postholder will:Play a vital role in the onward development, roll out and co-ordination of the new Single Point of Access (SPOA) Model in the management of Urgent Care. The SPOA model will co-ordinate home visit requests by referral from General Practices. The visits will be delivered by our Advanced Nurse Practitioner Team.This post will provide a confidential, comprehensive, and varied co-ordination role to Primary Care and Advanced Nurse Practitioner Services. The post-holder is required to use initiative and resource while exercising a high degree of confidentiality, diplomacy and tact. The post-holder is required to have proficient communication skills, including responding to referral requests from GP Practices. They are required to ensure that information is passed on accurately and timeously to the teams they are supporting.The post-holder operates within pre-determined procedures and parameters and is required to act independently, prioritising work with periodic review from line manager.Duration, Location, and Working Pattern: Secondments will be considered with line manager approval or Fixed Term for a period of 12 months.Job share will be considered.Location based in Greenock Health Centre. Full-time position of 36 hours per week. Shift pattern for this post is Monday – Friday, covering from 8.30am. Key ResponsibilitiesUnder the general direction of the management of the Primary Care Team the post-holder will beexpected to undertake most or all of the following responsibilities:-General Provide a high quality administrative and secretarial service to the multi-disciplinary team. Deal with telephone and face-to-face enquiries as received, providing information as required. Direct and prioritise enquiries as appropriate. Receive visitors (including patients and members of the public) to the premises in a polite and helpful way in accordance with local procedures. Book rooms using electronic or paper formats and ensure that rooms are set up appropriately Using electronic patient management system (EMIS, CNIS, Trakcare, Clinical Portal), arrange home visit allocations for ANPs. Book interpreters.
