This Role Is Subject To
The Leaving Care Service provides a statutory service to young people aged 16-25 years who are, or have been, Looked After by Manchester City Council.
The service is delivered through a community hub model, ensuring young people receive coordinated, accessible, and holistic support within their local communities. The approach promotes independence, resilience, and long‑term stability by bringing together partner agencies, local resources, and trusted professionals around each young person.

The Personal Advisor will play a key role in supporting young people with their transition to adulthood and assist them to live an independent life in their community.
The Community Hub provides open access support to care experienced young people from aged 16 to ensure the right guidance, advice and assistance is there for our young people, for whenever they need us.
In Manchester, all care experienced young people are supported to lead a Happy, Healthy, Safe & Successful life and reach their potential – this support extends into adulthood also.
The Hub is supported by colleagues from Housing & Health as well as offering a gateway to Education, Employment and Training opportunities. & community & charity services.
In the Hub we know and expect all young people to experience big changes in life, and we are here to work through challenges and celebrate successes together.
