The OAFU (Open Access Follow Up) Nurse is responsible for delivering high-quality, compassionate, and patient-centred care to individuals following cancer treatment at Imperial College Healthcare NHS Trust.The role supports patients on stratified follow-up pathways, with a strong emphasis on health promotion, self-management, and holistic wellbeing.Working collaboratively with multidisciplinary teams, cancer CNS teams, Macmillan services, and primary care providers, the post holder will assess patients’ physical, emotional, and supportive care needs, ensuring timely signposting and referrals where appropriate. The role also involves telephone triage and remote monitoring of patients, escalation of clinical concerns, coordination of follow-up care, and maintenance of accurate patient records.The successful candidate will contribute to delivering evidence-based nursing care while promoting safe clinical practice, effective communication, and patient advocacy. They will support service development through participation in audits, quality improvement initiatives, and education of support staff and students. The post holder will also maintain professional standards in accordance with the NMC Code of Conduct and Trust policies.This role is ideal for a motivated and compassionate registered nurse with an interest in cancer care and survivorship, who is committed to delivering excellent patient experience within a supportive multidisciplinary environment.Delivery of high-quality, evidence-based nursing care to patients on a stratified follow up pathway.Telephone triage for remotely monitored patients, escalating concerns appropriately, supporting the tracking and follow-up of stratified patients, assisting with referral triage, and ensuring that investigations are ordered and completed in line with agreed protocols.Assessing, planning, implementing, and evaluating individualised patient care.Acting as a patient advocate to ensure patient needs and preferences are respected.Organising and prioritising workload effectively, maintaining accurate patient documentation, and ensuring nursing practice complies with Trust policies and procedures. Effective communication with colleagues and the wider multidisciplinary team to maintain a positive and collaborative working environment.Assessing the information and support needs of patients, relatives, and carers, providing health education where required, and supporting the learning and development of cancer support workers, nursing assistants, and students.
