Reporting to the Clinical Lead, this role will work very closely with allocated wards taking responsibility for supporting the ward staff in managing all discharge pathways and taking on complex cases, under the instruction of Senior Discharge Planners.You will be a vital part of the hospital’s discharge service working alongside Hampshire and Isle of Wight Healthcare, Hampshire County Council and Portsmouth City Council to ensure a smooth and safe discharge process for our patients.We welcome applications from those who are highly motivated, organised and can work autonomously and flexibly to meet the needs of the service. Experience / Understanding of the healthcare setting. Educated to NVQ level 2 / 3, BTEC and knowledge about a range of Trust policies and management proceduresBe able to learn and participate in Portsmouth Hospitals University NHS Trust computer systems. DesirableHealth care experience within a hospital setting. Experience in managing discharge planning processes. Care CertificateEssentialSkills and KnowledgeExcellent communication skills, both orally and in writing, ability to liaise with staff on all levels and adapt communication styles to circumstance. Excellent organisational skills both individual and collective and attention to detail. Excellent interpersonal skills and proven ability to work in a team and individually. Time management skills and ability to prioritise own workload and work to deadlines. Ability to prioritise competing demands within a busy environment and be a pragmatic problem solver.
