We are looking for a dedicated Channel Island Liaison Coordinator to join our centralised 2-person Channel Island Liaison Team (CILT) at Southampton Hospital, support patients from Jersey, Guernsey and Alderney throughout their inpatient and outpatient journeys at UHS.You will be part of a 2-person specialist team that is a crucial single point of contact for Channel Island patients and UHS Staff. Your role will make a genuine difference to the patient experience for hundreds of people travelling long distance overseas from the Channel Islands for care. Your work will include visiting clinical areas communicating with channel island patients at the bedside, meeting families, UHS staff, coordinating overseas travel, accommodation and taxis when required, air ambulance/medical escort collaboration and coordinating.You will be supporting radiotherapy patients from the Channel Islands and the IOW, and collaborating with other external agencies connected to the channel island liaison service at UHS. The role expects patients to receive a high-quality of support from CILT throughout their stay and that all administrative and travel processes are adhered as per UHS Trust and States of Jersey/Guernsey standards.Some key responsibilities for this role; Identify newly admitted Channel Island patients, meet them and support their care journey. Coordinate timely and safe discharges, including commercial flight travel, boat travel and air ambulance arrangements. Work closely with clinical teams to ensure discharge documents and “fit to travel” forms are completed and given to patients in a timely manner. Meet/communicate regularly with patients, relatives, travel teams, UHS and Channel Island health professionals. Request accommodation from the Channel Islands off island travel team for patients fit for discharge but not yet fit to travel. Support radiotherapy patients from Channel Islands and IOW. Contribute to service improvements and keep Channel Island guidance and resources up to date. Handle confidential documentation in line with Trust policies. A proficient communicator who can build strong relationships with internal and external partners Effective and strong organisational and coordination abilities. Able to work independently but also part of a team whilst managing competing priorities.
