Brief DescriptionThe Institute for Family Health operates family medicine residency training programs that includes operation of a family medicine inpatient service in affiliated hospitals. The role of the Care Team Assistant (CTA) is to support the inpatient attending physicians and residents on duty at the Mount Sinai Hospital family medicine inpatient service with clerical and clinical duties. The CTA plays an important role in coordinating safe care transitions for patients and has the opportunity to support research and quality improvement initiatives in the inpatient unit. The CTA works under the direction of the front-line ordering clinician but is ultimately supervised by the inpatient director and the inpatient faculty liaison.RequirementsPerform patient-related administrative, care-coordination and inter-visit tasks assigned by the medical team. The most common of these include:Participate in patient care rounds to extrapolate the clerical tasks from patient presentations.Adding patient to EPIC Haiku / Canto for billing.Help primary team with pre-authorizations, confirmation of pharmacies, and arranging delivery of prescriptions/medications.Work with medical team and social work to expedite patient discharges (early identification of transportation, home services, as well as other social work needs)Collect data for QI initiatives.Assist in recording and improving understanding of hospital metrics including DBN (discharges before noon) rates, barriers to DBN, IDP (implement discharge planning) rates, and readmission rates.Conduct discharge follow-up phone calls to patients.Improve primary care provider (PCP) communication by either directly notifying the PCP of his/her patient’s hospitalization by phone or by EPIC in-basket messaging.Obtain test results and records from referring hospitals/physicians and ancillary services.Facilitate inpatient tests, radiology exams, as well as schedule post-discharge appointments.Key Knowledge, Skills, AbilitiesThe Care Team Assistant must participate in training to use the electronic health record (EHR) and clinical training under the direct supervision of the attending faculty and inpatient faculty liaison. The CTA must meet the established markers for proficiency and competency initially and every 3 months thereafter.The CTA should be a well-organized individual who is a fast-learner and able to anticipate patient/team needs. The CTA must be proficient in English, and the individuals should be familiar with general medical terms, with the understanding that they will gain further knowledge through on-the- job training.
