Working as part of the Integrated Community Team, the post holder will combine a reduced clinical caseload with a strong focus on practice education, supervision, leadership, and quality improvement. The role supports staff to develop competence, confidence, and consistency in practice, ensuring safe, effective, and personcentred care in line with Trust values and national standards.

The post holder will work autonomously within the Integrated Community Team, providing expert clinical advice, leadership, and support to nursing staff across a defined geographical area. Approximately half of the role focuses on clinical practice, with the remainder dedicated to education, transformation, and workforce development activity.
Key responsibilities include managing and prioritising workloads, delivering competencybased supervision, supporting appraisal and performance development processes, and identifying learning and development needs within the team. The role includes supervising students, supporting newly qualified staff, and acting as a mentor and role model for highquality, evidencebased practice.
The Transformation Facilitator will actively contribute to service improvement through audit, evaluation, and participation in transformation projects, working closely with the Clinical Leads and other senior colleagues. Effective communication, partnership working, and risk management are essential components of the role, alongside a strong commitment to patient safety, clinical governance, and continuous improvement.
The role requires flexibility, the ability to work across multiple sites, and confidence in managing complex clinical, professional, and organisational situations.
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks. This will include mandatory training, systems training, and the allocation of equipment.
As part of the induction process, new starters will have the opportunity to meet the executive team, senior managers, and attend a number of dropin sessions focusing on engagement, health and wellbeing, and key processes.
Probationary Period
This post will be subject to a probationary period.
Internal applicants are exempt from the probationary period (unless currently partway through a probationary period or employed as bank staff).
High Cost Area Supplement Fringe Zone
This post attracts a High Cost Area Supplement (Fringe Zone), which covers areas just outside the M25, including parts of Surrey, Hertfordshire, Essex, and Kent.
The Fringe zone weighting is 5% of the basic salary, with a minimum payment of £1,346 and a maximum of £2,270 per annum, pro rata for parttime staff.
