Location: Ealing or Staines / Hybrid (with regular travel across Ealing and surrounding areas)Salary: £34,800-£40,000Contract: 12-month fixed term, full-time, 35 hours per week, Monday to FridayDBS Level: EnhancedAs a Transition & Housing Programme Officer, you’ll play a key role in supporting the delivery of the Supported Housing Transition programme, carrying out detailed casework, tenancy reviews and property assessments.You’ll be working closely with residents, capturing accurate information, and ensuring every decision is properly evidenced and recorded. This is a role where strong organisation, attention to detail and clear communication are essential, alongside a genuine focus on supporting vulnerable residents through change.Key Responsibilities Carry out detailed household tenancy reviews, including occupancy checks, support needs assessments and suitability for transfer to General Needs accommodation Undertake property and scheme-level reviews, gathering data to support appraisal, retention and disposal decisions Complete all programme documentation, including case notes, assessment forms, risk logs and supporting evidence Work directly with residents, providing clear information, guidance and support throughout the transition process Collect, record and analyse data to support accurate reporting and decision-making Support engagement with Local Authorities, including providing information and attending joint visits where required Assist with move-on planning, working with support providers to monitor progress and outcomes Maintain and update programme tracking systems, ensuring all actions and tenancy changes are accurately recorded Ensure all documentation and data is completed within required deadlines and quality standards Escalate safeguarding, welfare and safety concerns in line with organisational procedures Act as Data Steward for programme information, ensuring accuracy, quality and compliance Contribute to continuous improvement by identifying opportunities to improve processes and resident experienceMore About You Educated to GCSE level (or equivalent) in Maths and English (Grades A*–C), with a relevant degree or professional qualification desirable This role requires regular travel and a full UK driving licence and access to a car is essential Strong communication skills, with the ability to engage clearly and professionally with residents, including those with complex or additional support needs Experience in housing, casework, resident engagement or property administration, with an understanding of housing legislation, tenancy management and safeguarding Strong administrative and analytical skills, with excellent attention to detail The ability to manage confidential and sensitive information appropriately Good IT skills, including Microsoft Excel, Word and case management systems Excellent organisation skills, with the ability to manage a structured caseload and work independently A good understanding of working with vulnerable residents and supporting positive outcomes through structured processesChoosing us is easyAs an employee of A2Dominion, we want you to feel rewarded and have a choice of flexible benefits, salary exchange benefits, and rewards and discounts that work for you, wherever you are in your life.We offer a wide range of staff benefits.
