Job Overview

We are looking for an enthusiastic, hard-working individual who has the ability to work well within a small dedicated team delivering a high standard of ward co-ordinator support to Gate 10A

We are looking for someone who has a friendly outlook with excellent communication skills and someone who thrives in a fast-paced and very busy working environment. The role requires an elevated level of patient contact .

Main duties of the job

Applicants will need to be an excellent team-player as well as the ability to work independently. They will need to be flexible and able to use their own initiative working proactively and in collaboration with team colleagues to ensure tasks are managed well.

You will need to be able to demonstrate excellent IT skills with the ability to plan, prioritise and manage your time and problem solve effectively.

Detailed Job Description And Main Responsibilities

• The purpose of the Ward Coordinator role is to support the ward team in managing the patient’s journey from admission through to discharge, ensuring that discharge occurs at the earliest appropriate opportunity and that the patient has the necessary support and care to remain safe in their discharge setting.
• The Ward Coordinator will also support the smooth running of the ward’s administrative functions, such as ordering stores, supplies, and specialist equipment as needed.
• The post holder will work closely with the nursing teams, the Ward MDT and the Integrated Discharge Service (IDS) and system partners to ensure that the patient receives prompt care and any delays to treatment are escalated in a timely manner. They will ensure the discharge process is active, dynamic and efficient and that ward information systems are updated in real time.
• The post holder will support the ward teams to have early conversations with patients and families/ carers around discharge and use this information to support the person’s journey whist in hospital.
• By doing this, the post holder will ensure that people only remain in the hospital for as long as they need to and will optimise their length of stay, making sure ‘flow’ is maintained and beds are available for people that need them.

Education/Training/Qualifications

Essential criteria

• Educated to GCSE or equivalent

Desirable criteria

• Have a basic understanding of outpatient booking processes and/or a fundamental understanding of the NHS.

Work Experience

Essential criteria

• Experience of working as part of a team in a multi-disciplinary environment.
• Ability to demonstrate good customer care skills.

Desirable criteria

• Experienced in the NHS or similar organisation.
• Previous Outpatient and/or clerical experience.

Knowledge/Skills/Abilities

Essential criteria

• Excellent verbal and written communication skills
• Able to develop good working relationships amongst team, colleagues and staff.
• Able to work flexibly to meet the needs of the service.
• The ability to work to tight deadlines.

Personal Qualities/Special Circumstances

Essential criteria

• Willingness to learn new IT systems.
• Enjoy working in an extremely busy department.
• Someone who is a team player, always helpful, happy and willing to assist other teams.
