We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.The RoleWe provide continuity across in-patient and community-based secondary NHS mental health care that is based on the principles of equality, dignity and respect and which takes into account spiritual and cultural diversity.Within the eating disorder service, care is delivered by a multi-disciplinary team in collaboration with partner agencies. It is based on the best available evidence for practice and is planned and delivered in a manner which promotes personal choice. The clinical team works collaboratively with service users, carers and their representatives to ensure that care needs are identified and met, and through utilising group/individual therapies across community and in-patient settings, will offer a credible alternative to hospitalisation or facilitate early discharge wherever possible.All patients will be cared for by a named individual who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high-quality mental health care in an environment where patients feel valued and where confidentiality is respected.Through a short- to medium-term case management approach, the service will provide intensive support and interventions to clients and carers prior to, during and following discharge from an in-patient episode within our partner services, with an expectation of preventing relapse and for new referrals in-patient episode of treatment.Please note that all roles will be assessed for sponsorship eligibility in line with current Home Office requirements. You will be based at Florence Street Resource CentreKey ResponsibilitiesYour Duties will include telephone enquiries from all staff groups in the NHS, patients, relatives, carers and external agencies and the management of outpatient clinics.
