Our developing Community Frailty Service includes geriatric consultants, Specialty Doctors, a Consultant Practitioner, GPs, Advanced Frailty Practitioners (ACPs) and trainee ACPs.
The West Kent Home Treatment Service is an innovative MDT providing hospital‑level interventions in patients’ homes. The team delivers CGA, IV therapies, bedside diagnostics, medication reviews, and oxygen/nebuliser therapy where appropriate.
The service operates seven days a week, with team check‑ins at the start and end of each shift. Home visits are planned, with paired visits where needed. Trust vehicles are available, though access to a car is essential. You will work with extensive clinical equipment and a highly skilled team to provide a rapid two‑hour community response. There may also be opportunities to support community hospital cover, alongside involvement in QI and educational activities.

As a Specialty Doctor within Urgent Care Services, you will play a central role in delivering safe, high‑quality clinical care and supporting the development of an effective, patient‑centred service. You will undertake the clinical and administrative duties outlined in your job plan, maintain CPD for revalidation.
You will provide expert assessment, diagnosis and management for patients with complex needs across a range of settings, including clinics, community hospitals and home visits. You will use sound clinical judgement, safeguard vulnerable individuals, and escalate concerns appropriately. Maintaining accurate records, adhering to professional standards and ensuring compliance with mandatory training are essential aspects of the role.
You will contribute to high‑quality clinical outcomes by offering leadership within the MDT, providing second opinions, supervising junior colleagues and participating in governance, reflective practice and service development meetings. You will support the development of evidence‑based pathways, identify vulnerable patients, and contribute to effective care planning. Cover for colleagues across localities may be required.
You will contribute to audits, research, service improvement and the development of clinical practice in response to changing population needs. Leadership, adaptability and a commitment to continuous improvement are key to success in this role.
