As we strive to improve our services and spaces for our residents as set out in the ‘Ambitious for Sutton’ corporate plan, Sutton aims to shape the future of the borough and adapt to the needs of its residents to make our Borough a great place to live, work and raise a family.About The Role And Our Ideal Candidate Adult Social Care Mental Health Team Within our boroughwide specialist service, there are three social work teams – The Transition Team, The Learning Disability Team, and the Mental Health Team.The Mental Health Team’s purpose is to coordinate and fulfill the Council’s statutory social care responsibilities for individuals with complex mental health needs including but not limited to Care Act assessments and reviews, Safeguarding and Mental Health Act work. Our service consists of a welcoming management team (Team Manager and three Assistant Team Managers), Approved Mental Health Professionals, Social Workers and Enabling Independence Workers. Within the team we have hubs that focus on different areas of responsibility which include an AMHP hub, hospital discharge hub, complex case hub and a review hub.We are recruiting for 2 social workers to join our review hub focusing on our ambitious transformation project of reviews for those with eligible Care Act needs and S117 Aftercare. You will be working closely with our health colleagues as well as independently to review packages of care both in and out of borough to ensure we meet the needs of our residents in a creative and supportive way.You Will Be Joining a Welcoming, Supporting, Hard Working Team Who Are Committed To Ensuring The Needs Of Those Experiencing Mental Health Issues Are Met. In Addition You Will Have Access To: Regular case and personal supervision Team reflective learning and development sessions Whole service (Mental Health, Learning Disability and Transitions) development, training and support Carer development opportunities including potential sponsorship for training as an Approved Mental Health Professional, Best Interest Assessor and Practice Educator About You This position offers an excellent opportunity for individuals with outstanding communication and assessment skills. You should be able to work proactively, with initiative, manage complex cases, all whilst having a passion for supporting vulnerable residents whose voices are often unheard. We encourage applications from candidates with experience in delivering person-centred adult social care and expect the following from all suitable candidates: Must be registered with Social Work England and hold a degree in Social Care or similar. Have experience of working in a multidisciplinary team setting. Have an experience or strong desire to work with people with mental health challenges. Have experience and knowledge of reviewing social care plans and an understanding of S117 Aftercare. Ability to communicate complex social issues clearly to both patients and professionals.
