This is an exciting opportunity to be part of a newly established specialist team, playing a key role in delivering high-quality Continuing Healthcare assessments and decision-making. You will work closely with ICB Nurse Assessors to complete Decision Support Tools (DSTs), alongside undertaking Care Act assessments, reviews and support planning for adults with complex and often rapidly changing needs.The role requires strong partnership working across multi-disciplinary teams, ensuring that individuals receive appropriate, timely, and person-centred care packages, with clear alignment between health and social care responsibilities. You will manage a complex caseload, providing robust analysis of needs across CHC domains, contributing to eligibility decision-making, and ensuring care packages are reviewed and remain proportionate and effective.A key part of the role is to ensure CHC processes are delivered in line with the National Framework for NHS Continuing Healthcare, supporting lawful, consistent, and defensible decision-making while maintaining a strong focus on outcomes for residents.About The TeamHaringey’s Continuing Health Care Team is a newly established, specialist service within Adult Social Care, created to strengthen and embed high‑quality CHC practice across the borough. The team is made up of skilled practitioners who work at the interface of health and social care, supporting residents with complex, long‑term health needs.
