We are looking for a motivated & enthusiastic individual who will be part of a multidisciplinary team working with patients in their own homes to provide holistic care. Your primary responsibility will be to assist with enabling people to maintain their independence at home and supporting people after they have left hospital. The post holder needs to be flexible and able to use their own initiative as well as provide high standards of patient care in support of the team to ensure delivery of a high quality service to the local community.Experience not required, although of benefit.
