We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.The RoleRenfrewshire Community Treatment and Care (CTAC) service is looking for 3 full time band 3 health care support workers to join our team on an 18 month fixed term contractThe CTAC Service will provide an accessible, appropriate, high quality, culturally sensitive effective treatment room service to individuals within a primary care treatment & care room setting whilst also working in partnership with other wider members of the primary care team.The post holders will work collaboratively under the remit of the CTAC service and assist in skilled clinical interventions and the implementation and evaluation of programmes of care for patients and their carers. You will support the Community Treatment Room Nursing Staff/ District Nurse or their deputy to deliver safe and effective patient care within a community/ GP clinic setting, mainly without direct supervision. The service also provides a Phlebotomy service to residents of Renfrewshire within a clinic environment and within their own homes contributing to health improvement initiatives for both individuals and familiesThe post holders will be required to work flexibly in terms of location geographically within the HSCP to support other areas of teams working within the local CTAC / District Nursing service.A Clean, Current Full Driving Licence Is RequiredIt is desirable that you have an SVQ Level 3 in healthcare or equivalent experience and are a car driver. The shift pattern for these posts is over 5days.Duration, Location, and Working Pattern: Location based in Renfrewshire HSCP CTAC TeamFixed-term Contract for 18 monthsFull-time position (36 hours per week)Shift Pattern for this post are 8.15 until 4.15pm, Monday-Friday, excluding PHsKey ResponsibilitiesTo assist with the nursing care of the patient under the direction and instruction of a trained nurse. This will include all basic care within the patient’s home and in clinics. Including skin care, application of prescribed creams and eye drops, prompt patients to take prescribed medication; and undertakes non complex dressingsTo provide care and support to patients and their families in their own homes and within clinics, often working alone To observe and report any change in the condition of the patient and communicates this verbally to the nurse in charge. Changes must be documented in patient recordsTo administer bolus feed via a gastrostomy tube and commence / discontinue PEG feeding systems as directed by trained nurse. To change the water in a gastrostomy tube in adultsTo participate and support the care of patients ensuring skin integrity is maintained, adhering to current manual handling procedures and assists those patients unable to self-care with personal hygiene and dressingTo support patients and carers with catheter management and stoma care and maintenance.
