We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.The RoleThe Home First Response Team provides timely, person-centred interventions that support patients to maximise their health, wellbeing, and independence through a coordinated, multidisciplinary approach.The team acts as a key link between acute and primary care services, ensuring effective communication, seamless care planning, and optimisation of the patient journey across care settings.Duration, Location, and Working Pattern:This is a permanent full-time post (36) working these hours 5 over 7 days. You will be based at the Royal Alexandra Hospital. Please note that all roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.Key ResponsibilitiesWithin the Home First Response Team, you will utilise highly developed specialist knowledge, underpinned by advanced theory, clinical practice, and experience, to deliver a high-quality specialist service.
