The ideal candidate brings proven remote assessment experience and drives results from day one.You willFollowing national guidance and the policies developed by ProblemShared, use independent prescribing skills and knowledge to complete health assessments, initiate medication and titrate dosageFormulate and implement plans for the treatment and medication management of clients using NICE guidance in line with national/local formulariesEnsure that the client’s interests are fully considered within the context of their treatment plan, ensuring that all parties actively participate in ethical, shared decision making, utilising a multidisciplinary team approachProvide ongoing medication reviews for your ADHD patients (adults and children)Actively manage patient care coordination, including shared care transitions and referrals to local NHS services for annual reviewsVerify that patient physical observations (height, weight, blood pressure, pulse) have been recorded prior to their medication review appointmentComplete risk assessments and review safeguarding needs alongside treatment strategies during ongoing patient careEnsure accurate and timely client records are kept on the clinical platform.
