We are looking for somone with an interest in system working, to support a programme of work with partners across the Leeds Health & Care system to deliver our local ambition to develop a Neighbourhood Health approach.You will work as part of a small team delivering a programme of work in relation to Active Recovery & the Stroke pathway. This work includes the expansion and strengthening of the existing Active Recovery model and a project to implement an early supported discharge pathway for survivors of stroke. On occasion you may also provide project support to a programme testing a new model of integrated, community-based specialist diabetes care.We welcome applications on a secondment basis from anyone applying from within the Leeds Health & Care Partnership. Roles are offered on a fixed-term basis until the end of March 2027 and may be subject to further extension in 2027/28.We are open to applications from people looking to work between 22.5 and 37.5 hours per week (0.6-1.0 wte).You will work on behalf of the Leeds Health & Care system, hosted by the Leeds Community Healthcare Business, Change & Improvement Service – a network of skilled and experienced project managers, change agents and business managers.
