Halo Case Management Ltd are seeking an Associate Case Manager to join our growing team.This is an excellent opportunity for a motivated and compassionate professional with experience in health, social care, rehabilitation, or support work who is looking to develop a career in case management.The role involves supporting clients with complex injuries and disabilities, coordinating rehabilitation programmes, attending multidisciplinary meetings, liaising with families and professionals, and assisting senior case managers with the delivery of client-centred support.We are looking for someone who is organised, proactive, professional, and able to work independently while maintaining high standards of communication and record keeping.Essential:• Excellent communication and organisational skills• Full UK driving licence and access to a vehicle• Ability to work flexibly and travel as required• Experience within health, care, rehabilitation, or support settingsDesirable:• Previous case management experience• Relevant clinical or health/social care qualificationCompetitive rates offered depending on experience.To apply, please send your CV and covering letter to Helen@halocm.co.uk
