An exciting and challenging opportunity has arisen for a fixed term contract (9 months with likelihood of extension to 12 months) Band 7 Occupational Therapist to join our Discharge to Assess service who support complex patients following discharge from Homerton and out of area hospitals returning home to Hackney and the City of London.This post would be ideal for a dynamic occupational therapist to provide assessment and rapid review of people in their homes to review functional ability, ensure their safety at home and prevent re-admission to hospital. The role also includes providing senior advice/joint working with acute ward therapists at Homerton Hospital as well as promotion of single-handed care and the Discharge to Assess philosophy within the trust.The Discharge to Assess Service is part of the established Integrated Independence Team which places the service user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the health system.The team provides Rapid Response, Intermediate Care Rehab, Frailty Virtual Ward and Rapid Care services and has a comprehensive multidisciplinary professional team ready to provide support. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.The ideal candidate will have strong experience as an acute or community occupational therapist and will be familiar with the therapy and care needs of people leaving hospital and returning to their own homes.You should have excellent communication skills, a proactive approach, prioritisation and problem solving skills.You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from senior occupational therapy staff and experienced multi disciplinary staff across the team.Supervision and in service training are carried out on a regular basis and you will have an annual appraisal.
