NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.The RoleThe Long-term Conditions service is an AHP-led, digital-first self-management service. Patients referred to the team have been diagnosed with a long-term health condition which has impacted their quality of life and ability to work. Common conditions could include ME/CFS, fibromyalgia, post-viral fatigue and Long Covid, etc.Care is provided via a 6-week virtual self-management group, specialised workshops and targeted 1-1 interventions (often via near me). This is a hybrid role; therefore, work will be undertaken remotely from home, through community home visits, and by attending one of our remote working bases (Rowanpark or Belmont) or local health centres, as required. The Long-term Health Conditions Service works across 6HSCPs within NHSGGC boundaries.
